Forms

There are several forms associated with participating in Special Olympics, whether you are an athlete, coach or volunteer.  Below is a list of forms needed for Lee County Special Olympics.  If you have any questions regarding any of the forms listed below, please contact Dana Stewart, Lee County Director of Special Olympics for more information.

Bowling Practice Score Sheet (DUE Feb. 27, 2015):

This form should be used by an athlete, coach or teacher when turning in qualifying scores for an athlete.  Athletes are required to have a minimum of 15 practice scores in order to compete in the County Bowling Tournament.  If you do not have time to finish up your scores at the local bowling alley, remember you can practice in your school gym.  The dimensions for the bowling lane are:  From foul line to the pins is 60 feet long and the lane is 42 inches wide.  Please remember: Rails, also known as bumpers, cannot be used by an athlete.  If an athlete is competing using a ramp (also known as Ramp Assisted Bowling), please indicate this on your score sheet.  Also, remember to indicate shoe sizes for all athletes on the form.  All forms should be mailed to Lee County Special Olympics, Attention: Dana Stewart, 425 Perry St., Auburn, AL, 36830, faxed to 334-501-2960, dropped off at the Parks and Recreation Administrative Offices in Auburn, or sent via e-mail to Dana Stewart

Bowling Tournament T-Shirt Order Form (DUE Feb. 20, 2015):

This form should be used by a coach, teacher or parent when ordering shirts for the upcoming County Bowling Tournament.  Athletes, Teachers, Coaches, Parents, friends and supporters can purchase a t-shirt for $10.00 each.  All forms should be mailed to Lee County Special Olympics, Attention: Dana Stewart, 425 Perry Street, Auburn, AL, 36830, faxed to 334-501-2960, dropped off at the Parks and Recreation Administrative Offices in Auburn, or sent via e-mail to Dana Stewart

Track and Field Entry Forms (DUE March 20, 2015):

These forms should be used by an athlete, coach or teacher when turning in qualifying times/distances for an athlete.  Please note: there are 3 pages to the Track and Field Score Sheet (Page 1, Page 2, Page 3).  Page 3 lists all of the event codes needed to complete the form.  Athletes may compete in two (2) events plus a relay event; however, they cannot compete in three events or more.  Please read the event codes (Page 3) carefully!  For example, if you have a wheelchair athlete he/she cannot be listed with a code of 2, which is the 100M Dash.  Instead, the athlete would be listed with a code of 17 or 18, depending on if the wheelchair is motorized or non-motorized.  All forms should be mailed to Lee County Special Olympics, Attention: Dana Stewart, 425 Perry St., Auburn, AL, 36830, faxed to 334-501-2960, dropped off at the Parks and Recreation Administrative Offices in Auburn, or sent via e-mail to Dana Stewart

Track & Field Meet T-Shirt Order Form (DUE Feb. 20, 2015):

This form should be used by a coach, teacher or parent when ordering shirts for the upcoming County Track and Field meet.  Each athlete, teacher and coach participating in the 2014 Lee County Special Olympics Track and Field meet will receive a FREE t-shirt (Please limit 2 teacher t-shirts per class.)  Parents, friends and supporters can purchase a t-shirt for $10.00 each, but a separate order form must be filled out and received by the deadline.  All forms should be mailed to Lee County Special Olympics, Attention: Dana Stewart, 425 Perry St., Auburn, AL, 36830, faxed to 334-501-2960, dropped off at the Parks and Recreation Administrative Offices in Auburn, or sent via e-mail to Dana Stewart

2015 Track and Field Meet Information:
The 2015 Lee County Track and Field Meet will be held on Wednesday, April 1, 2015 at the JD Evilsizer Track in Smiths Station, AL.  (Rain Date is Thursday, April 9.)  The event will run on Central Time and athletes must report at 8:30 a.m., Opening Ceremonies at 9 a.m. and the event begins at 9:30 a.m.

Application for Participation in Special Olympics (also called the Medical Form) DUE Jan. 5, 2015:
Medicals forms are required for all participants in the Special Olympics program.  Medicals are current for 3 years, after which a new medical form must be completed.  A physician's signature is required on the medical form; a nurse practitioner's signature will not be accepted.  If you are unsure if your students need to update their medicals, please contact
Dana Stewart, Lee County Special Olympics Co-Director to check against the Master List.  All medicals must be current on the day the student participates in an athletic event.  This form is in triplicate and therefore cannot be downloaded as a PDF document.  Please contact the Co-Director to receive a form via mail.

Official Special Olympics Release Form (also called the Waiver) DUE Jan. 5, 2015:
Waiver forms are required for all participants in the Special Olympics program.  Waivers must be completed every year until the athlete reaches the age of 18 years.  At this time, an athlete can complete the waiver on his/her own and the waiver will stand.  If you are unsure if your students need to update their waivers, please contact Dana Stewart
, Lee County Special Olympics Director to check against the Master List.  All waivers must be current on the day the student participates in an athletic event.  This form is in triplicate and therefore cannot be downloaded as a PDF document.  Please contact the Co-Director to receive a form via mail.

Application for Sports Training Certification (also called the Coaching Application):
If you are interested in becoming a Certified Coach in one or more sports, then you must complete a Coaching Application, attend a General Orientation session, a Training Seminar that is specific to the sport you are interested in, and spend a minimum of 10 hours working with a Special Olympics athlete(s) in that sport.  General Orientation sessions, as well as sport-specific training seminars are held year round and all over the state.  If you are interested in attending one of these sessions or seminars, please contact the Lee County Special Olympics Co-Director or check the Coaches/Volunteers section of this website.  This form is in triplicate and therefore cannot be downloaded as a PDF document.  Please contact the Director to receive a form via mail.

Volunteer Registration Form, Class A:
If you are interested in volunteering at a practice or event, or chaperoning a trip, then you must complete a
Volunteer Form.  The form asks for sensitive information, such as your driver's license and your social security number, in order to complete a full background check on all volunteers.  Please provide this information and please note: we will protect your information and your privacy.

Packing List for State Games:
The Packing List for the 2015 State of Alabama Summer Games at Troy University will be posted in May 2015.
